Advance Care Directives
An Advance Care Directive is an important way of letting people know your preferences about your healthcare and treatment in case you are seriously ill or injured and not able to make decisions. Having an Advance Care Directive will make it easier for those significant to you and health staff if they need to make decisions for you. Making an Advance Care Directive is an important part of Advance Care Planning.

TERMS & CONDITIONS
Priority of Access - CHSP service staff and volunteers will be given registration priority.
Payment (only if applicable) - STARS will send an invoice for the training to the organisation for payment within 2 weeks.
Confirmation – Confirmation of registration will be by phone or email. When a training session is fully booked a waiting list will be created.
Cancellations – STARS will attempt to ensure that all courses take place. However STARS reserves the right to alter or cancel any session/s if required. We will notify you as soon as possible.
Transfers - Transfers between volunteers or staff from the same organisation may be permitted (upon request) for the same course. Transfers to another course are only permitted when the course has been cancelled by STARS.
Non-attendance of registered participants may incur a cost recovery fee if less than 5 working days notice is provided.
Certificates of Attendance will only be issued when a person attends the full training session.
