Short Term Pathways
In addition to ongoing home care services, you may be eligible to access three short-term pathways that help older people stay independent and live at home longer:
• Restorative Care Pathway
• Assistive Technology and Home Modifications (AT-HM) Scheme
• End-of-Life Pathway
Restorative Care Pathway
The Restorative Care Pathway provides short-term, intensive support to help you stay independent and continue doing the things you enjoy.
This pathway aims to:
• Prevent or delay the need for ongoing in-home or higher-level care
• Support you to regain your ability to carry out daily activities
• Help you manage new or changing age-related conditions
• Provide education and skills to maintain independence for longer
Access to the Restorative Care Pathway is based on an assessment of your needs.
Assistive Technology and Home Modifications (AT-HM) scheme
The AT-HM scheme gives Support at Home participants access to assistive technology and home modifications based on assessed needs without needing to save up funds from their quarterly budget.
Assessors may approve funding for assistive technology, home modifications or both.
Examples of assistive technology:
• Mobility equipment such as walking sticks, frames and wheelchairs
• Toileting supports including bedpans and commodes
• Bathing devices such as shower chairs and bath boards.
Assistive Technology and Home Modifications list (AT-HM list)
Home modifications
Home modifications make your living space safer and more accessible, allowing you to stay at home longer.
Examples include:
• Grab rails in the shower or bathroom
• Internal and external handrails
• Non-slip surfaces on floors and stairs
In some cases, more significant modifications may be approved, such as:
• Bathroom redesigns to improve accessibility
• Ramps or stair lifts
End-of-Life Pathway
The End-of-Life Pathway provides additional in-home support for people in their last three months of life, helping them remain comfortably at home.
It is available to:
• Current Support at Home participants, and
• Older people not currently receiving Support at Home services
This pathway offers urgent, short-term funding for higher levels of in-home care, which can be used instead of your standard Support at Home budget.
Eligibility:
• A medical or nurse practitioner has provided an estimated life expectancy of three months or less
• A score of 40 or less on the Australian-modified Karnofsky Performance Status (AKPS) (a measure of mobility and frailty)
